London Social Ventures is partnering with Care City, a not-for-profit driving innovation in health and social care across North East London.

We’re supporting the application of university-led innovations in real-world adult social care settings.

Together, we’re inviting researchers to explore practical solutions to the question:

How do we help people stay well, in the place they call home, for as long as possible?

Timeline

What you’ll get

  • Join a community of researchers, innovators and social care practitioners tackling one of society’s most pressing challenges
  • Support to apply your solution in real-life settings, including:
    • Access to pre-identified partner sites
    • Mentorship in adapting your solution to frontline contexts
    • Connections to local partners for delivery and feedback.
  • Continuous feedback and mentorship through the programme
  • Structured end-of-programme review session to synthesise learnings from your pilot, explore opportunities and gaps in your model, and consider next steps for scaling and development

Who should apply?

This initiative is open to academics or researchers employed at a London university

We’re looking for ideas that are already developed, even at an early stage.

Your solution might be:

  • A research-backed solution with a working prototype that addresses one of our challenge areas, even if not yet piloted OR
  • A product or tool that’s been piloted successfully in another sector, but could help solve a social care challenge as listed in our challenge areas

How to apply?

To apply, all you need to do is fill out our expression of interest form (required time 5-10 mins) by 11 August. Our team will review these applications and schedule structured conversations with shortlisted applications to understand your solutions further.

These structured conversations will take place throughout August on a rolling basis.

The Challenge

How do we help people stay well, in the place they call home, for as long as possible?

Prevention

How can we stop people’s health or wellbeing from getting worse in the first place?

  • Helping people manage long-term conditions (like diabetes or heart issues)
  • Keeping people active, mentally and physically
  • Reducing loneliness or social isolation
  • Supporting people to take their medication properly

Independence

How can we help people do everyday things on their own, for as long as possible?

  • Making it easier to move around, get dressed, cook, or wash
  • Helping people feel confident leaving the house
  • Reducing the burden on family carers
  • Supporting people to live at home instead of going into care too early

Prediction

How can we spot problems early – before they become a crisis?

  • Tracking signs of health decline (e.g. mobility, appetite, falls)
  • Using data to find out who might need help soon
  • Including things like housing, income or loneliness in risk tools
  • Identifying and helping carers get support before burning out

Productivity

How can we make delivering care simpler and more efficient?

  • Better ways to plan staff rotas or visits
  • Cutting time spent on admin or travel
  • Helping different professionals work together
  • Reducing stress and turnover for care staff

Hospital Discharge

How can we help people get home from hospital safely and quickly?

  • Supporting recovery before and after operations
  • Avoiding delays due to paperwork or finding appropriate services
  • Helping people stay well at home and avoid being readmitted
  • Giving doctors and patients confidence in managing health after an incident/ hospital admission

Assessment and Planning

How can we make it quicker and easier to get care in place?

  • Making it easier for people to find info and tools to arrange care themselves
  • Reducing time spent on admin for staff
  • Helping people find the right equipment or tech when they need it
  • Making care plans flexible to the changing needs of people
